Histopathologic Endocrine Study on Endometrium in Ectopic Pregnancy.

Abstract
OBJECTIVE
Our purpose was early detection of ectopic pregnancy using endometrial pattern changes and serum beta-hCG. METHODS: We had observed in 158 patients of ectopic pregnancy at the Department of Obstetrics and Gynecology, Gachon Medical college, Gil Medical Center hospital from July 1, 1993 to January 31, 1995. The patients were evaluated by the distribution of age, number of induced abortion, gestational periods, serum beta-hCG, and patterns of endometrium. RESULTS: The mean age distribution was 28.4 years old (+/-SD 4.83). The distributionon of the mean number of induced abortion was 1.3. The patterns of endometrium were consisted of secretory phase 39.9%, decidual reaction 24.7%, proliferative phase 16.5%, Arias-Stella reaction 13.9%, and mixed phase 5.0%. The endometrial patterns excluding decidual reaction and Arias-Stella reaction were 61.3% of which were proliferative phase, secretory phase, and mixed phase. Decidual reaction and Arias-Stella reaction were more prevalent in 5~7 gestational age weeks, but after 8weeks they were sharply declined. And the trend of decidual change as well as Arias-Stella reaction were correlative in 5~7 gestational age weeks. Decidual reaction and Arias-Stella reaction were more prevalent in group of beta-hCG 100~10,000 mIU/ml. But There was no statistical significance due to wide range of deviation (unpaired student`s t-test: p>0.05). CONCLUSION: Additional endocrine tests, ultrasonography and diagnostic endoscopy are mandatory for early detection of ectopic pregnancy in association with endometrial biopsy because neither the decidual reatction or Arias-Stella reaction nor the proliferative phase, secretory phase, or mixed phase are not tentitive finding of the ectopic pregnancy.
